O início da pandemia de COVID-19 foi marcado por incertezas diante do desconhecimento sobre a doença. Uma série de dúvidas relacionadas ao uso de imunobiológicos no contexto da pandemia foi levantada, inclusive em relação ao tratamento com omalizumabe em pacientes com urticária crônica (UC). Este estudo teve como objetivo analisar os dados relacionados à gravidade da COVID-19 e a evolução da urticária em pacientes em terapia com omalizumabe acompanhados por especialistas no Brasil. Foi realizada análise retrospectiva de dados de pacientes com UC tratados com omalizumabe entre julho/2020 e junho/2021 que apresentaram COVID-19. Foram avaliados dados relacionados às características clínicas dos pacientes e evolução da urticária durante a infecção pelo SARS-CoV2. Foram incluídos 28 pacientes em tratamento com omalizumabe, sendo 27 com urticária crônica espontânea (UCE), dos quais 25% tinham alguma urticária induzida associada. A maior parte dos pacientes (71%) estavam utilizando doses quadruplicadas de anti-histamínicos modernos de 2ª geração associados ao omalizumabe. Todos os pacientes estavam com os sintomas controlados. Entre os sintomas apresentados durante a COVID-19, os mais frequentes foram: febre (43%), cefaleia (36%), mal-estar (32%), hipo/anosmia (29%) e tosse (21%). Quatro pacientes foram hospitalizados, um deles em unidade de terapia intensiva. Um paciente relatou piora dos sintomas da UC durante a COVID-19. Cinco (18%) pacientes apresentaram piora dos sintomas da UC após a resolução da COVID-19. Todos os pacientes se recuperaram da COVID-19 sem sequelas graves. O OMA não pareceu aumentar o risco de COVID-19 grave e poderia ser usado com segurança em pacientes com UC.


The beginning of the COVID-19 pandemic was marked by uncertainty due to lack of knowledge about the disease. Questions were raised about the use of immunobiologicals in the pandemic context, including omalizumab for patients with chronic urticaria (UC). This study assessed COVID-19 severity and the clinical course of urticaria in Brazilian patients on omalizumab therapy who were monitored by specialists. We retrospectively analyzed data from chronic urticaria patients treated with omalizumab between July, 2020 and June, 2021 who presented with COVID- 19. Clinical characteristics and the course of urticaria during SARS-CoV2 infection were analyzed. The sample consisted of 28 patients treated with omalizumab, 27 of whom had chronic spontaneous urticaria (UCE) and 25% of whom had associated chronic inducible urticaria. Most of the patients (71%) were using quadruple doses of second-generation antihistamines associated with omalizumab. The symptoms of all patients were controlled. The most frequent symptoms during COVID-19 were: fever (43%), headache (36%), malaise (32%), hypo/anosmia (29%) and cough (21%). Four patients were hospitalized, including 1 in intensive care. One patient reported worsening chronic urticaria symptoms while infected with COVID-19. Five (18%) patients experienced worsening chronic urticaria symptoms after recovery from COVID-19. All patients recovered from COVID-19 without serious sequelae. Omalizumab did not appear to increase the risk of severe COVID-19 and can be safely used in patients with chronic urticaria.

Os anti-inflamatórios não esteroidais (AINE) são os fármacos mais frequentemente associados a reações de hipersensibilidade (RH) na prática clínica. Na parte 2 dessa atualização sobre as RH aos AINE, discutiremos os aspectos clínicos dessas reações, com foco nos sinais e sintomas, como diferenciar os fenótipos clínicos, fazer a orientação desses pacientes e quando indicar procedimentos complementares, como testes cutâneos, de provocação e dessensibilização.


Nonsteroidal anti-inflammatory drugs are a major cause of drug hypersensitivity reactions in clinical practice. In this "Update Part 2", we discuss the clinical picture, including the main signs and symptoms, how to distinguish clinical phenotypes, how to manage affected patients, and when to indicate additional procedures, such as skin testing, challenge, and desensitization.

ABSTRACT Introduction: Systemic Mastocytosis comprises a group of neoplastic diseases characterized by clonal expansion and infiltration of mast cells into several organs. The diagnosis and treatment of this disease may be challenging for non-specialists. Objective: Make suggestions or recommendations in Systemic Mastocytosis based in a panel of Brazilian specialists. Method and results: An online expert panel with 18 multidisciplinary specialists was convened to propose recommendations on the diagnosis and treatment of Systemic Mastocytosis in Brazil. Recommendations were based on discussions of topics and multiple-choice questions and were graded using the Oxford Centre for Evidence-Based Medicine 2011 Levels of Evidence Chart. Conclusion: Twenty-two recommendations or suggestions were proposed based on a literature review and graded according to the findings.

ABSTRACT

Os anti-inflamatórios não esteroidais (AINE) estão entre os medicamentos mais utilizados no mundo e são os fármacos mais frequentemente associados à ocorrência de reações de hipersensibilidade na América Latina. As reações têm grande variabilidade de apresentações clínicas e, consequentemente, com abordagem terapêutica difícil. Nesta revisão, abordamos aspectos farmacológicos dos AINE, bem como as definições, epidemiologia e fisiopatologia das reações de hipersensibilidade aos AINE. Por fim, discutimos aspectos genéticos associados à intolerância e alergia a esses fármacos.


Nonsteroidal anti-inflammatory drugs (NSAIDs) are among the most commonly used medications worldwide and the drugs most frequently associated with the occurrence of hypersensitivity reactions in Latin America. The clinical presentation of the reactions varies widely, which makes them difficult to treat. In this review, we address pharmacological aspects of NSAIDs, as well as the definitions, epidemiology, and pathophysiology of hypersensitivity reactions to NSAIDs. Finally, we discuss genetic factors associated with intolerance and allergy to these drugs.

A urticária crônica é uma condição que afeta mais de um milhão de brasileiros, com grande impacto na qualidade de vida. Mesmo com diretrizes bem difundidas para o seu diagnóstico e tratamento, seu manejo pode ser desafiador em pacientes pediátricos, idosos e gestantes. Para auxiliar o médico especialista nestes casos, o Departamento Científico de Urticária da Associação Brasileira de Alergia e Imunologia elaborou esta revisão com as principais dúvidas e dificuldades referentes ao tema nestes grupos de pacientes.


Chronic urticaria is a condition that affects more than a million Brazilians with a significant impact on quality of life. Although there are well-established guidelines for diagnosis and treatment, the management of chronic urticaria may be challenging in pediatric, older, and pregnant patients. With the purpose of helping specialists manage these cases, the Urticaria Scientific Department of the Brazilian Association of Allergy and Immunology prepared this review with the most common doubts and difficulties about this topic in those patient groups.

A urticária aguda é uma causa frequente de consulta com alergistas, caracterizada por urticas e/ou angioedema. Embora autolimitada e benigna, pode causar desconforto significativo e raramente representar uma doença sistêmica grave ou reação alérgica com risco de vida. Nesta revisão, elaborada pelo Departamento Científico de Urticária da Associação Brasileira de Alergia e Imunologia, foram abordadas as principais questões referentes ao tema para auxiliar o médico especialista e generalista.


Acute urticaria is a frequent cause of consultations with allergists, being characterized by wheals and/or angioedema. Although self-limited and benign, it may cause significant discomfort and uncommonly represent a serious systemic disease or life-threatening allergic reaction. In this review prepared by the Urticaria Scientific Department of the Brazilian Association of Allergy and Immunology, the main questions about this topic are addressed to help specialists and general practitioners.

ABSTRACT

A urticária aquagênica é uma forma rara de urticária crônica induzida (UCInd) desencadeada por um estímulo específico. A patogênese não é totalmente compreendida, mas os sintomas se iniciam minutos após a exposição cutânea à água, independentemente de sua temperatura, e as urticas têm o padrão foliculocêntricas. O diagnóstico é confirmado através do teste de provocação, e o tratamento de primeira linha são os anti-histamínicos de segunda geração. Neste artigo, relatamos um caso de urticária aquagênica e fazemos uma breve revisão da literatura sobre o tema.


Aquagenic urticaria is a rare form of chronic inducible urticaria (CIndU) triggered by a specific stimulus. Pathogenesis is not fully understood, but symptoms appear minutes after cutaneous exposure to water, regardless of temperature, and wheals have a folliculocentric pattern. The diagnosis of CIndU is confirmed by provocation testing using established protocols, and first-line treatment is second-generation antihistamines. In this article, we report a case of aquagenic urticaria and provide a brief review of the relevant literature.

Abstract Objectives The study describes a case series of hereditary angioedema with C1 Inhibitor Deficiency (C1INH-HAE) in order to corroborate six clinical warning signs "HAAAAE (H4AE)" to enable early identification of this disease. Methods The authors analyzed the C1INH-HAE cohort to analyze the clinical aspects of the present study's patients and corroborate the six clinical warning signs of the Hereditary Angioedema Brazilian Guidelines. Data regarding demographics, the onset of disease, time to diagnosis, frequency of attacks per year, organs involved, triggers, crisis duration and their outcomes, and disease treatment were collected. Then the authors developed an acronym, H4AE, to help healthcare professionals remember the warning signs. Results The authors included 98 patients in the study, with a mean age of 38.1 years, 67.3% being female, and 75.3% with a family history of HAE. HAE diagnosis was delayed, on average, 13.7 years after its initial manifestation. Exploratory laparotomy was reported by 26.9%, and orotracheal intubation by 21.3% of the present study's patients; 61.3% and 30.3% of them were admitted at least once in the hospital and in the intensive care unit, respectively. The authors constructed an acronym "H4AE" with the six warning signs of HAE: Hereditary, recurrent Angioedema, Abdominal pain, Absence of urticaria, Absence of response to antihistamines, Estrogen association. Conclusion C1INH-HAE is still underdiagnosed and associated with high morbidity. The study showed clinical features of this disease, corroborating the warning signs, which may be useful in raising awareness and improving the diagnosis of C1INH-HAE. The authors suggest the acronym "H4AE" to remind the warning signs.

Há o empenho contínuo de especialistas no desenvolvimento de tratamentos resolutivos ou eficazes nos controles das doenças, no entanto, a entidade urticária crônica espontânea (UCE), quando refratária à primeira linha de tratamento, os anti-histamínicos, apresenta um prognóstico desfavorável. Existe um arsenal de medicamentos biológicos disponíveis já consolidados como eficazes e seguros, porém eventualmente nos defrontamos com a inacessibilidade a estes medicamentos, devido aos custos dos mesmos e aos trâmites necessários para dar início ao tratamento. Tais fatos fundamentam a discussão sobre terapias alternativas com outros fármacos, visando manter o manejo adequado da doença e a qualidade de vida dos pacientes.


Specialists have made a continuous effort for the development of effective treatments for disease control; however, chronic spontaneous urticaria (CSU), when refractory to the first line of treatment, ie, antihistamines, has an unfavorable prognosis. There are biological medicines available, which have been consolidated as effective and safe, but we are occasionally faced with a lack of access to these medicines due to their costs and the necessary procedures to start treatment. Such facts support the discussion about alternative therapies with other drugs, aiming at maintaining the adequate management of the disease and the quality of life of patients.

Com o início do programa de vacinação contra a COVID-19 no Brasil, surgiu uma série de questionamentos relacionados ao uso dos imunizantes em pacientes com doenças imunoalérgicas. Neste documento, o Departamento Científico de Urticária da Associação Brasileira de Alergia e Imunologia (ASBAI) se posiciona revisando as principais dúvidas relacionadas à imunização para COVID-19 em pacientes com urticária.


As the COVID-19 vaccination program started in Brazil, many questions have arisen regarding the use of vaccines in patients with immune-allergic diseases. In this document, the Scientific Department of Urticaria of the Brazilian Association of Allergy and Immunology takes a stand by reviewing the main queries regarding COVID-19 immunization in patients with urticaria.

Introdução: Os sintomas gerados pela urticária crônica (UC) afetam significativamente a qualidade de vida dos pacientes, e o estresse pode ser um fator de exacerbação. Isso se torna ainda mais importante no atual cenário de pandemia da doença causada pelo coronavírus (COVID-19). Diante da impossibilidade de manter a mesma quantidade de consultas presenciais, surgiu a necessidade de saber como estariam os pacientes com UC: se apresentariam exacerbação da doença de base caso se infectassem com o SARS-CoV-2, se a UC predisporia os pacientes a um quadro mais grave de COVID-19, e se o estresse emocional a que os pacientes estariam sujeitos exacerbaria sua doença de base. Métodos: Trata-se de um estudo observacional retrospectivo com dados coletados através do registro das informações coletadas de pacientes com UC durante remarcação de suas consultas, através de ligações telefônicas. Resultados: Foram incluídos 140 pacientes no estudo, no período de 29/04/2020 a 15/07/2020. O estresse emocional estava presente em 80 pacientes (57,1%), sendo que destes, 30% relataram piora da urticária. A obesidade foi a outra comorbidade mais relatada pelos pacientes com UC (35%). Dos 22 pacientes que procuraram o Pronto-Socorro, 9 (40,9%) foram investigados. Destes, 5 (55,6%) realizaram investigação específica para COVID-19. Conclusões: Durante a pandemia da COVID-19, os nossos pacientes com UC se encontraram mais estressados emocionalmente, e isso foi um fator associado à piora da urticária. A obesidade, no nosso grupo de pacientes, foi muito prevalente.


Introduction: Chronic urticaria (CU) symptoms significantly affect patient quality of life, and stress can be an exacerbating factor. This becomes even more important in the current pandemic setting of the novel coronavirus disease 2019 (COVID-19). Given the impossibility of maintaining the same schedule of in-person medical appointments, there was a need to know how patients with CU would behave: if they would have an exacerbation of the underlying disease if they became infected with SARS-CoV-2, if CU would predispose patients to a more severe form of COVID- 19, and if emotional stress would exacerbate their underlying disease. Methods: This is a retrospective observational study of data collected from records of patients with CU when their medical appointments were rescheduled via telephone call. Results: One hundred and forty patients were included in the study from 4/29/2020 to 7/15/2020. Stress was present in 80 patients (57.1%), of which 30% reported worsening of urticaria. Obesity was the most reported comorbidity in patients with CU (35%). Of the 22 patients who sought emergency care, 9 (40.9%) were investigated. Of these, 5 (55.6%) underwent specific investigation for COVID-19. Conclusions: During the COVID-19 pandemic, our CU patients were found to be more emotionally stressed, and this factor was associated with worsening of urticaria. Obesity, in our group of patients, was very prevalent.

Introdução: A asma é uma doença respiratória crônica e os vírus respiratórios são gatilhos bem conhecidos das suas exacerbações. O coronavírus pode se manifestar com sintomas pulmonares. Objetivo: Avaliar o comportamento clínico dos pacientes com asma durante a pandemia de COVID-19. Métodos: Estudo retrospectivo de prontuário eletrônico de pacientes adultos asmáticos, em acompanhamento em serviço terciário e que receberam ligações telefônicas para reagendamento, nos meses iniciais da pandemia de COVID-19. Foram analisados dados demográficos, sintomas de asma, atopia, comorbidades e sintomas relacionados à infecção pelo coronavírus. Os pacientes foram classificados conforme a história de crise de asma, como também steps de tratamento da asma. Resultados: Foram incluídos 207 pacientes, sendo 79,7% do sexo feminino, com média de idade de 53,3 anos e tempo de asma de 35 anos, sendo 81,7% atópicos. As principais comorbidades foram obesidade (32,9%), hipertensão arterial (47,3%), diabetes mellitus (17,4%) e estresse emocional (68,1%). Do total, 87 pacientes (40,1%) apresentaram sintomas agudos, sendo que 20 (9,7%) procuraram pronto atendimento, e 15 (7,2%) foram investigados para COVID-19, todos negativos. Apenas 7 pacientes (3,4%) exacerbaram e necessitaram de corticoide sistêmico. Dentre os pacientes com sintomas respiratórios agudos, os sintomas mais frequentes sugestivos de COVID-19 foram dispneia, tosse, astenia e cefaleia, quando comparados com os pacientes que não apresentaram sintomas agudos de asma (p < 0,05). Conclusão: Este estudo observou que os pacientes asmáticos apresentaram baixa prevalência de exacerbação da asma no período da pandemia pelo coronavírus. Os pacientes com sintomas agudos podem ter sido subdiagnosticados para COVID- 19, devido à baixa procura ao pronto atendimento. Antecedente de atopia pode funcionar como fator protetor para COVID-19 em pacientes asmáticos.


Introduction: Asthma is a chronic respiratory disease, and respiratory viruses are well-known triggers for asthma exacerbations. Patients with coronavirus disease 2019 (COVID-19) can present with pulmonary symptoms. Objective: To evaluate the clinical behavior of patients with asthma during the COVID- 19 pandemic. Methods: A retrospective study of electronic medical records of adult asthmatic patients being followed up in a tertiary care service and who received telephone calls for rescheduling during the COVID-19 pandemic. Demographic data, asthma symptoms, atopy, comorbidities, and symptoms related to coronavirus infection were analyzed. Patients were classified according to their history of asthma attacks. Results: In total, 207 patients were included; 79.7% were female, mean age was 53.3 years, mean asthma duration was 35 years, and 81.7% were atopic. The main comorbidities were obesity (32.9%), high blood pressure (47.3%), diabetes mellitus (17.4%), and emotional stress (68.1%). Eighty-seven patients (40.1%) had acute symptoms, of which 20 (9.7%) sought emergency care and 15 (7.2%) were investigated for COVID-19, all of which were negative. Only 7 patients (3.4%) had exacerbations and required systemic corticosteroids. Among patients with acute respiratory symptoms, dyspnea, cough, asthenia, and headache were the most frequent complaints suggestive of COVID-19 when compared to those without an asthma attack (p < 0.05). Conclusion: This study found that asthmatic patients had a low prevalence of asthma exacerbation during the coronavirus pandemic. Patients with acute symptoms may have been underdiagnosed for COVID-19 in view of the low demand for emergency care. Previous atopy may act as a protective factor for COVID-19 in asthmatic patients.

ABSTRACT

A urticária crônica é uma doença com grande impacto socioeconômico e na qualidade de vida do indivíduo. O adequado conhecimento de formas de tratamento eficazes e com perfil de segurança satisfatório, assim como dos mecanismos preditores de resposta ao tratamento, são essenciais para que se alcance um controle adequado da doença. O omalizumabe é um anticorpo monoclonal anti-IgE com eficácia reconhecida e bom perfil de segurança no tratamento da urticária crônica. Objetivamos elucidar questões envolvidas no manejo desta medicação, através da revisão em literatura de estudos atuais e com relevância clínica. Foi realizado levantamento de questões importantes e pouco elucidadas, buscando respostas baseadas nestes estudos. Com isso, foram abordados aspectos práticos do tratamento com o omalizumabe, esclarecendo desde os fenótipos dos pacientes e conduta adequada para estas diferentes situações, trazendo possíveis fatores preditores de resposta ao tratamento e contemplando também um novo anticorpo monoclonal anti-IgE no manejo destes pacientes.


Chronic urticaria is a disease with great socioeconomic impact on people's quality of life. Adequate knowledge of effective treatments with a satisfactory safety profile as well as of the predictive mechanisms of response to treatment are essential for achieving adequate disease control. Omalizumab is an anti-IgE monoclonal antibody with recognized efficacy and a good safety profile in the treatment of chronic urticaria. We aim to elucidate issues involving the management of this medication through a literature review of current studies with clinical relevance. A survey of important and unclear questions was conducted, and the answers were sought in the studies. Thus, practical aspects of omalizumab treatment were addressed, including the phenotypes of patients and appropriate approaches for different situations. Possible predictive factors of response to treatment and a new anti-IgE monoclonal antibody for the management of these patients were also reported.

Introdução: A urticária colinérgica (UCol) é um subtipo de urticária crônica induzida, desencadeada pela sudorese e o aumento da temperatura corporal. A associação de UCol com atopia é referida como um possível subtipo mais grave. A manifestação de angioedema estaria associada a um quadro mais prolongado de urticária e a sintomas extracutâneos, por exemplo, anafilaxia. Objetivo: Avaliar a frequência de atopia e/ou angioedema nos pacientes com UCol, em um centro terciário. Métodos: Estudo retrospectivo de prontuários de pacientes com UCol acompanhados em um centro terciário. Todos apresentavam teste de provocação para UCol positivo. A frequência de atopia e/ou angioedema foi avaliada nestes pacientes, como também as características gerais nestes subgrupos. Resultados: Foram incluídos 30 pacientes, sendo 60% do gênero feminino e idade (média) de 32,9 anos e tempo de doença (média) de 7,5 anos. O angioedema foi referido por 8 pacientes (26,7%), não foram observadas diferenças significantes entre os dois grupos (com e sem angioedema). Em relação à atopia, 9 pacientes (30%) realizaram a investigação através de IgE específica para aeroalérgenos, sendo positivo em 6 destes (66,7%). Embora sem diferença estatística, o grupo de pacientes com UCol e atopia apresentava valores de IgE sérica total mais elevados e maior frequência de associação com outras urticárias induzidas. Conclusões: Neste estudo, a frequência de atopia foi elevada e associada a níveis elevados de IgE sérica total. O angioedema foi relatado em mais de um quarto dos pacientes, independente da associação com UCE, favorecendo a uma maior gravidade à UCol. Doze pacientes (40%) não responderam aos anti-histamínicos, apesar da dose quadruplicada, sendo necessários outros esquemas terapêuticos.


Introduction: Cholinergic urticaria (CholU) is a subtype of chronic induced urticaria that is triggered by sweating and increased body temperature. The association of CholU with atopy is referred to as a possible subtype but more severe. The manifestation of angioedema is believed to be associated with more prolonged urticaria and extracutaneous symptoms such as anaphylaxis. Objective: To assess the frequency of atopy and/or angioedema in patients with CholU in a tertiary care center. Methods: A retrospective study of medical records of patients with CholU followed-up at a tertiary care center was conducted. All patients had a positive test for CholU. The frequency of atopy and/or angioedema was assessed in these patients, as well as the general characteristics in the subgroups. Results: Thirty patients were included in the study; 60% were female, mean age was 32.9 years, and mean disease duration was 7.5 years. Angioedema was reported by eight patients (26.7%). There were no significant differences between the two groups (with and without angioedema). Concerning atopy, nine patients (30%) underwent investigation using specific IgE for aeroallergens, with six positive results (66.7%). Although there was no statistical difference, the group of patients with CholU and atopy had higher total serum IgE values and a higher frequency of association with other induced urticaria. Conclusions: In this study, the frequency of atopy was high and associated with high levels of total serum IgE. Angioedema was reported in more than a quarter of patients, regardless of the association with ECU , favoring greater severity of ChoIU. Twelve patients (40%) did not respond to antihistamines, despite the quadrupled dose, requiring other therapeutic regimens.

ABSTRACT

A urticária crônica espontânea (UCE) é uma condição rara, benigna e com sintomas que afetam adversamente a qualidade de vida, tanto dos pacientes quanto de seus familiares, visto que ainda não existe tratamento resolutivo. O manejo farmacológico de primeira linha consiste no uso de anti-histamínicos em doses licenciadas ou até quadruplicadas, e na ausência de resposta ao anti-histamínico, os consensos mundiais recomendam, na sequência, a adição de omalizumabe. Ambos são amplamente utilizados e considerados seguros e eficazes. No entanto, ainda há alguns questionamentos acerca da anti-IgE: quando e como suspender a medicação, por quanto tempo usar ou quando retornar o uso da mesma, caso haja recidiva. Logo, alguns artigos foram revisados visando melhor elucidação dessas dúvidas.


Chronic spontaneous urticaria (CSU) is a rare, benign condition with symptoms that adversely affect the patients' and their families' quality of life, as there is still no curative treatment. First-line pharmacological management consists of the use of antihistamines in licensed or even quadruplicate doses and, if there is no response to the antihistamine, worldwide consensus recommends subsequent addition of omalizumab. Both are widely used and considered safe and effective. However, there are still some questions about anti-IgE, including when and how to stop the medication, how long it should be used, and when to resume using it in the case of a relapse. Therefore, some articles were reviewed to facilitate the elucidation of these questions.
